Field Trip Permission Slip Planning Checklist
A start-to-finish checklist for building, sending, collecting, and filing the permission slips for one school trip.
Reviewed for 2026, updated September 8, 2026.
Most permission slip problems are timing problems. The form goes home too late, the reminder goes to everyone instead of the families who have not answered, and the last few slips arrive on the morning of the bus. This checklist puts every step in order, from confirming trip details with the office to filing the signed records after the trip, so nothing depends on memory during a busy week.
Work through it once per trip. The first block happens before anything is sent, the middle blocks cover sending and collecting, and the last block covers trip day and what happens afterward. It works whether you collect on paper, with a general online form, or with a dedicated tool. Pair it with our Permission Slip Deadline Planner to set the send and reminder dates, and with the guide at /guide/ for the reasoning behind each line.
- Confirm the date, times, destination, travel method, supervision plan, and cost with the office before writing the form.
- Start from the school's saved template and update only the trip-specific block: date, venue, transport, cost, and deadline.
- Check that the consent language and media release match current district policy for this school year.
- Keep the form short: activity details, emergency contact, allergies and medications, media release, and chaperone interest.
- Confirm the nurse already holds medication plans so the slip only asks whether anything has changed.
- State clearly that typing a name counts as a signature and that the parent agrees to sign school forms electronically.
- Set a return deadline several school days before the bus count or venue headcount is due.
- Put the deadline and what happens if a family does not respond in the first line of the message.
- Pull the current roster and note each family's preferred channel, language, and second household if there is one.
- Send through at least two channels, typically text and email, with a plain link that opens without an app or login.
- Leave a small printed stack at the front office for families who prefer paper.
- Send at a time parents are likely to see the message, not late on a Friday afternoon.
- Track every student on one list with clear statuses: not sent, sent, viewed, signed, declined, needs follow-up.
- Enter paper returns on the same list the day they arrive so nobody is counted twice or missed.
- Send one reminder around the midpoint, only to families who have not responded.
- Send a final reminder one or two days before the deadline, again only to non-responders.
- Call or speak in person with the last few families rather than sending a fourth message.
- Move incomplete slips, such as a missing emergency contact or an unsigned medical section, to needs follow-up instead of counting them as done.
- Record a declined response as a real answer and arrange supervision at school for students who are not going.
- On trip day, print or pull up one list with contacts and medical notes for the teacher, the office, and the nurse.
- Confirm the final headcount matches the bus list, the chaperone ratio, and the venue reservation.
- After the trip, export or file every signed slip by school year and trip with a cover sheet listing each student's status.
- Restrict access to emergency and medical details once the trip is over.
- Note what slowed returns this time and update the template before the next trip.
